Artemios Mintzas, 61, is accused of peppering a busy Sydney shopping strip with bullets in October, leaving one person seriously injured and sparking a shootout with officers.
The incident in Croydon Park, in the city's inner west, plunged residents into an hours-long lockdown after up to 100 bullets were sprayed and bullet holes were left in nearby cars.
Mintzas appeared by audio-visual link from prison before Burwood Local Court on Wednesday where Judge Jennifer Price noted he had been hit with a further 23 charges.
His barrister Dymphna Hawkins told the court she was unaware the new charges had been laid against her client.
"I was not informed, Your Honour," she told the judge.
She told the court that prosecutors had previously told her some charges could be added and some may be withdrawn but no decision had been made at the time.
Mintzas now faces 48 charges in total, including 23 counts of shoot with intent to murder, 17 counts of discharging a firearm with the intent of causing grievous bodily harm, and one count of discharging a firearm to avoid arrest.
The matter was adjourned until July 15.
Prosecutor Kartini Saddington told the court the solicitor in charge of the matter was on holiday and could not sign off on the charge certificate containing the finalised alleged offences.
Ms Hawkins applied to have prosecutors pay her client's costs after yet another adjournment.
However, Judge Price rejected this application noting the barrister would have had to appear before court anyway due to the fresh charges.
Outside court, Ms Hawkins told reporters she was not surprised by the new allegations against her client.
She was asked how Mintzas was doing in prison.
"I don't know anybody who's really particularly good in jail," she said.
The 61-year-old is accused of engaging in a shootout with NSW Police during the October shooting spree after officers were allegedly targeted.
No police officers were hurt but over a dozen people had minor injuries, mostly from broken glass.
One man underwent surgery after being shot in the neck while riding in the back seat of a car.
The shots were allegedly fired from the window of a unit above a shop during a nearly two-hour stand-off.
Mintzas was arrested after tactical officers stormed the unit, where they allegedly seized a rifle and ammunition.
