Lancaster progressed to a preliminary final after winning its second final against Avenel by 13 points.
A low-scoring affair saw the Wombats up 16-7 at half-time, and only two goals would be kicked after the main break.
Lancaster jumped out to a 25-8 advantage by three-quarter time, then held off the Swans late despite going goalless in the fourth term, coming out as 3.9 (27) to 2.2 (14) winners.
Nicholas McAuliffe, Thomas Withall, Nathan Oliver, Xavier Kellow, Kurtis Seabrook and Jarom Owen-Horn were the Wombats’ best in the victory.
They will now face Murchison-Toolamba in a preliminary final, the winner of which takes on Nagambie in the decider.
Under-15
Stanhope is into the grand final after a 41-point thrashing of Tallygaroopna.
The Lions set themselves up during a solid first half before kicking clear in the third quarter, with Jaxan Pitt impressing with three goals.
Tally will now play Avenel in a preliminary final, the winner taking on Stanhope for the premiership.

Girgarre is into the grand final after beating Shepparton East 35-31 in a qualifier.
It was an impressive win for the Kangaroos considering they were the underdog, having finished second to the Eagles in the regular season with three fewer victories.
The club named Jayde Ilsley and Erin Potter as the side’s best-on in the game via its Facebook page.
In the other final, Stanhope cruised to a 51-35 win over Violet Town.
The victory means the Lions will play Shepparton in a preliminary final this Saturday, the winner taking on Girgarre in the decider.
C-reserve
Girgarre went down to Tallygaroopna in a qualifying final, being largely outmatched in a 54-36 loss.
The Kangaroos will now play Merrigum in a preliminary final, with the Bulldogs progressing after a 37-32 win over Lancaster.
17-and-under
Stanhope won its second post-season match, holding off Violet Town and progressing to a preliminary final after a 46-35 victory.
The Lions will take on Tallygaroopna for a spot in the grand final, where Shepparton East awaits.
15-and-under
Rushworth booked its ticket for the grand final after a surprising 45-23 thrashing of Tallygaroopna.
The other semi-final saw Girgarre up against Stanhope, with the Kangaroos eliminating the Lions by triumphing 39-29.
Girgarre now faces Tallygaroopna in a preliminary final this Saturday.
13-and-under
